Depends on the build of the game you are playing. I noticed in the last and current beta that your wife will not get pregnant if other members of your clan are in the same place getting freaky. So if one of your brothers and his wife are in the same place then your brothers wife will probably get pregnant instead of your own.
I married Ira hoping for a more complex family dynamic by becoming heir to the ruler of an existing faction. I already did the 'make your own faction and conquer everyone" thing in Warband. I think Taleworlds will flesh this out so let's encourage that and ask them to keep working on this aspect. #11.
